How to become a member?

Regular members

• Shall include all regular and probationary employees of XU and its affiliate units such as Xavier Science Foundation (XSF), Research Institute for Mindanao Culture (RIMCU), Xavier University Community Multi-Purpose Cooperative (XUCMPC) and Loyola House and retirees of Xavier University.

Associate Member A

• Composed of alumni of Xavier University.

Associate Member E

• Composed of relatives of the Regular, Associate A and S members aged 18 years old and above.
• Relatives are defined as: spouse, children, siblings (and their spouses – if applicable, and/or their children), and parents (natural and in-laws.)

Associate Member S

• Composed of student 18 years old and above.

Requirements for Membership

A member must have complied with the following requirements:

1. Attendance in Pre-Membership Education Seminar (PMES) PMES Schedule is on every first Saturday of the month


2. Accomplished application for membership, membership information card, subscription agreement. For subscription agreement, please download and fill in the correct form for your membership type. Please see list below:

3. Submit the following together with the filled out membership forms to XUCCCO Office:
   
   

   Regular Member

   • Certificate of employment from XU HR
   • Photocopy of any two (2) ID’s: SSS, Passport, Driver’s license, COMELEC, Postal, Employee ID
   • Two (2) pcs of 1 x 1 picture
   
   

   Associate Member A

   • Photocopy of any proof of Alumni: Alumni ID, Diploma, Transcript of Record
   • Photocopy of any two (2) ID’s: SSS, Passport, Driver’s license, COMELEC, Postal
   • Two (2) pcs of 1 x 1 picture
   
   

   Associate Member E

   • Photocopy of any proof of consanguinity or affinity
   • Photocopy of any two (2) ID’s: SSS, Passport, Driver’s license, COMELEC, Postal
   • Two (2) pcs of 1 x 1 picture
   
   

   Associate Member S

   • Photocopy of XU ID
   • Photocopy of any two (2) ID’s: SSS, Passport, Driver’s license, COMELEC, Postal
   • Two (2) pcs of 1 x 1 picture


4. BIR TIN Number


5. Certificate of completion of the prescribed Pre-Membership Education Seminar (PMES);


6. Paid the membership fee of One hundred fifty pesos (P150), initial share capital of One hundred pesos (P100) and initial savings deposit of One hundred pesos (P100)


7. To become a full-pledge member, paid the required minimum share capital of:
   
   
   • Regular Member - Six thousand pesos (P6,000)
   • Associate Member A - Five thousand pesos (P5,000)
   • Associate Member E - Five thousand pesos (P5,000)
   • Associate Member S - Five thousand pesos (P5,000)
